I think I have become a stronger critical thinker. I’m more mindful about how I am assessing and communicating. I am more efficient about how I gather information and communicate effectively with others to figure out solutions to complex problems. My critical thinking is just that, it’s “mine” so it’s self-directed, self-disciplined, self-monitored, and self-corrective thinking (The Foundation for Critical Thinking, 2019).
The biggest factor in developing my clinical reasoning skills has been applying my knowledge to clinical cases like patient care and have the opportunity to reflect on decisions. I’ve also been able to connect symptoms with diagnosis and figure out that maybe something else is going on with my patient then what was originally diagnosed. I’ve become more confident in my reasoning and with the guidance of peers have been able to address these new symptoms and or possible diagnosis with physicians.
Along with the peer review, case review and developing of my clinical reasoning skills, this has also further developed my clinical judgement. Taking the time to reflect with my peers or manager when an unexpected clinical event happens, like death or near-miss
An important thing I’ve had to remind myself is that: learning to think critically is a lifelong journey, and there’s always more to learn.
Describe an example of how a nurse can use clinical reasoning and judgment with quality improvement practices.
Since safety is a top priority, nurses can use their critical thinking, clinical reasoning to help develop and apply safety and quality improvement with clinical judgement (Leibold, 2020). The process of getting to a decision is using clinical reasoning while the final decision is the clinical judgement.
An example would be a nurse using her clinical reasoning to realize a patient that has a foley catheter doesn’t need it anymore and could potentially save the patient from CAUTI if it was removed. This would not only prevent CAUTI but also would make the patient have to get up and either walk to the bathroom or commode, making them get their muscles moving, improving strength and balance and by getting up and moving could prevent pneumonia.
How does evidence-based practice relate to clinical reasoning and clinical judgment?
Evidence-based practice involves basing clinical decisions and practice on the best available evidence. The reasoning helps the nurse make judgements about the relevance of particular research and clinical evidence for a specific patient and setting.
Describe an example of how communication relates to clinical reasoning and clinical judgment.
Clinical reasoning occurs within social relationships or situations involving patient, family, community, and a team of health care providers. The nurse situates themselves within all the relationships and socially engages to attribute to critical reasoning.
